ELIGIBILITY:
Class Instructors registered in the program by the Agency, who teach or lead classes or programs that are promoted, organized, conducted, and supervised by SCMAF member agencies are eligible.
PERIOD OF ELIGIBILITY:
Eligibility will commence on the date the SCMAF Class Insurance Program registration application is approved by the insurance underwriter. This is generally within five (5) working days that the application is received by the SCMAF Office.
FEES:
$1.60 per participant with $5,000 excess medical benefits for the term of the class. The fee includes the $1 million liability coverage, per occurrence with a $3 million annual aggregate limit.
CLASS INSURANCE PROGRAM BENEFITS:
Instructors registering their classes will receive the following: $5,000 excess medical benefits for the instructor and all participants with $1 million liability, per occurrence, insurance for the instructor, the agency, and the facility owner.
ELIGIBLE YOUTH AND ADULT ACTIVITIES:
Youth and adult classes where participants are formally registered prior to their commencement in the program, with the exception of sports competition programs, are eligible. Note: Sports programs of instruction and sports clinics and camps are eligible under the heading of sports development. A class is a single subject/activity for which participants are registered for a designated period of time. Each new enrollment or commencement date will constitute a new class. In case of ongoing enrollment classes, the date of coverage will begin with the first date of SCMAF Recreation Insurance Program registration and continue to the next sign up period (maximum of 12 weeks). The number of participants will be defined as the number of individual students who have or will enroll in any given class. For ongoing classes for open enrollment, individuals signing up for classes in a given calendar year will be counted, even if an individual only signs up for and attends one class period.
INFORMATION REQUIRED:
All Agencies enrolling in the SCMAF Class Insurance Program must provide:

Completed Application

Two (2) copies of the Agency Class Brochure or Catalog with class titles, name of the instructor(s); start and end dates for the class; days and times of the class; location of the class

Recent claims or losses for the activities

Projected number of students enrolled in the classes

Sample copy of the class instructor contract or agreement

Copy of the participant liability waiver form in use for the class
WAIVERS:
All participants in classes insured under the SCMAF Class Insurance Program must have signed a waiver of liability and release form with the participating agency.
PAYMENT:
Payment may be included with the registration forms or an agency may request an invoice from SCMAF. All invoices will be due in thirty (30) days.
AUDITS:
As SCMAF does not require that the class roster or class registration be included with the forms, SCMAF retains the right to audit the enrollment records of any classes registered in the SCMAF Class Insurance Program.
